Eligibility Requirements
 
Finding out if you are eligible for Home Care services is simple.  Here we will provide you with all the information you need to know on how our Home Care services can work for you.

 
Primary Home Care, Community Attendant Services, and Family Care Services
 
PHC, CAS, and FC services are available to eligible Medicaid clients whose health problems cause significant limitations to activities of daily living. The patient's physician prescribes all services.  CAS services are for individuals who do not have social security. FC services have no task requirements nor does it need a practitioner's statement for services.

Community Based Alternative
 
Available to eligible clients 21 years and older and also referred to as the Nursing Facility Waiver Program, (NFW).  The client must have a necessity determination for nursing facility care and must be determined at risk for nursing facility placement.

Private Pay
 
Privately funded PAS services are very much like the PHC and CBA programs. This program is available for anyone who is able to pay privately for the services.
