Community Care
Save Home Care, Inc. provides a variety of home care services. These services are for clients whose health problems limit them from performing daily activites like food preperation, cleaning, laundry, grooming, etc. To view each service home page, click on the links below. This program is a nontechnical, medically related personal care service that is open to Medicaid-eligible adults and children whose health problems cause them to be functionally limited in performing activities of daily living according to a practitioner's statement of medical need. PHC services are provided by an attendant.
PHC provider agencies also provide Community Attendant Services and Family Care services. These services are also nonskilled, nontechnical services provided to eligible clients who are functionally limited in performing daily activities. Family care does not require a practitioner's statement of medical need.
This program is for clients who do not have social security to cover services. |

Family Care is a nonskilled, nontechnical attendant care service for eligible adults who are functionally limited in performing activities of daily living. Primary Home Care provider agencies have the option of providing these services. Family Care services are provided by an attendant and do not require the supervision of a registered nurse.
With FC you do not need a doctor's statement for services. |
